Regardless of your reason for changing names, the first step is updating the Social Security Agency. Other agencies, such as the IRS, DMV and US Passports will only change your name to match what is currently on file with the SSA. There is no fee to change names with the SSA. Once your name change has been recorded you will be mailed a replacement SSA card. It has the same number, but will show your new name. There are a number of ways to apply:
Online (recommended)
We recommend applying online at https://www.ssa.gov/personal-record/change-name. If your name change record and identity records have been digitized and shared with agencies, then you may complete the application online without needing to present any documents in person.
If your identity can’t be verified online, you will be directed to complete your name change in person at an SSA location. The site will instruct what original documents you can take to a branch.
Apply in person
If you don’t want to start online, you may instead file the SS-5 paper form in person at an acceptance facility. Be prepared to show full evidence of identity and citizenship documents as listed on the form.
Apply by mail
Mailed applications are possible, but not recommended as the SSA require original identification documents. These will be returned to you in the same manner that they were received (eg: standard or tracked mail). The SSA prefers not to receive original identification documents by mail as you may be without these for some time.
Changes made with the SSA are visible to other agencies within 3 working days.
